Know a Child in Need?​

Due to the number of children in need of a CHD Tablet Talk package, referrals must come through the hospital social worker, child-life specialist, or a partnered nonprofit organization. Any referral application that is not made by a nonprofit organization, child-life specialist, or hospital social worker will not be considered. Fill out the form below to make a referral today. Once the application is reviewed to identify the child/facility in need, we work to fulfill units with devices that can be assigned to a patient during their hospital stay, or in certain circumstances, given to the child indefinitely through direct sponsorship/fundraising.  Disclaimer: Application is not a guarantee of care package provision as all referrals go through a needs-based screening process.​
